OPTIONAL BENEFITS
Supplemental Accident provides up to $500 of first dollar benefits of first dollar benefits and treatment of an injury within 90 days of an accident.
Maternity Benefit comes in increments of $1,000, $2,000 or $3,000, During the first benefit year, the plan will pay up to 50% of the maximum; up to 75% during the second year and 100% in the succeeding years.
Prescription Card allows you to purchase generic prescription drugs for $15 co-payment and name brand drugs for $25 co-payment.
